If using the PDF document: when you print, look at Page Scaling, and select either
"Fit to Printable Area" or "Shrink to Printable Area" (this will keep the form on one page).
If using the Word document, please save the form to your computer.
If you would like to pick up a form in person, go to the
KOFE Radio Station, located at 201 N. 8th St., St. Maries, ID.
You may also drop off completed entry forms at the KOFE Radio Station.
